Do I have to disclose any prior criminal activity to the New York DOS?

You will have to notify the New York Department of State if you have been convicted on a felony, if you have any pending charges, or if you have ever had a license revoked, suspended, or denied. You will also have to provide the required supporting documentation in order for the DOS to evaluate your case.
